It is awarded to those wounded in battle, even if the wound is considered to be 'self inflicted', if it is considered to happen in the 'heat of battle'. If you throw a grenade, as Dole did, and it comes back and wounds you, you can get the Purple Heart. If you struggle with an enemy soldier, and in that struggle you accidentally shoot yourself, you can get the Purple Heart.
