In Kerry's case at least one purple heart "injury" was not signed off on by the commanders in his company at the time of his initial request. He waited until they were reassigned and then ran it through the new commander. By the time he had put in a couple specious requests, it seems that his commander (and fellow crew) would have just wanted the whiner out of his hair.
