I attended college with a person who was motivated and probably close to average intelligence. His life was consumed by his desire to become a doctor. He did well in the classes he had to and eventually was accepted into medical school. I lost track of him, but suspect he made it through and probably passed his boards.

He is probably an acceptable doctor somewhere today. I think the author acknowledged that people like this can make it.
