I've read that Medicare is actually a very cost efficient and effective method of healthcare delivery, esp. considering that those enrolled, being older, are using a disproportionate amount of healthcare.

I'm pretty sure almost all of Medicare's cost efficiency is that they pay much lower rates for all services, which is why lots of Doc's won't take Medicare patients. What you have is all the private insurance patients subsidizing the care provided to Medicare patients. That is nothing to expand to cover the whole population.
