Medicare not insurance....just called that. The govt. picks up most of the tab, not a for-profit insurance. So with regard to Univ. coverage, the debate must first be between whether having a govt. agency write the checks to the providers or having a for-profit ins. company write the checks or some combination of the two. Next step, of course, is to decide how much money it would take and where would that pool of money come from. And of course the type and degree of coverage to provide, etc.
