Your own body kills you. In the Spanish Flu Pandemic people literally drowned in their own body fluids.

That Pandemic was unusual in that healthy people were among those most at risk. Normally it's the very old, very young, and the sickly that are victims. With the Spanish Flu your own immune system turned on you, and having a strong immune system was a disadvantage.

That was one of the very few times it paid to be an out of shape slob. Hmmmmmm, maybe H5N1 won't be a problem for most americans after all! ;)
