Two problems with body counts. One, how to distinguish between those who were fighting and those who were just in the wrong place at the wrong time?

You don't need to - the idea is not to measure bad guys, it is to measure stability. The surge is supposed to bring stability. If people continue to die in record numbers inside "cleared and held" areas, it doesn't matter if they were fighting or shopping, the surge could be seen as failing in its objective.

Two, when a scoring system is in place, there is pressure to inflate the numbers.

In this case the pressure would be to deflate the numbers. It measures regions AFTER they are cleared and held. Any deaths after the place is cleared and held are bad, since they indicate a lack of stability.
