How do the insurance companies decide what's covered? Someone told me they pay for fertility drugs and in vitro fertilization for heaven's sake.

Health 'insurance' is just another way to make money. Health insurance companies take your premiums and then figure out creative ways to not pay for anything.

I've heard that some companies actually have a policy to reject claims X number of times with the expectation that most people will simply give up. They need to be banned as the inefficient and corrupt leeches they are.

At any rate, health care is headed for disaster in the US and I don't think we're looking more than a few years in the future.
