I don't think their medical director has to actually be present. Can be a phone link, etc.

Apparently an NP can prescribe lots of things. No appointments necessary plus you buy tires and fruit on the way out the door.

I see the health care system fraying around the edges and people finding many opportunities for profit. The flip side are doctors who charge wealthy patients a 'retainer' to be on call for them. They have many fewer patients so they have more time.
