Sam,

agree that nicotine causes cancer

I was under the impression that nicotine on it's own has not been proven to be a cancer causing agent.. only the delivery mechanism.. cigarettes etc.. I know that there is speculation to the contrary but nothing definitive that I have found..

Sam,
Things have come around since Silent spring, and because of it millions have died:

Stossel did a piece on dioxin a few years back, and doing a search I found lots of sites shooting down his report, but not the report itself. I did find this:
junkscience.com
wired.com

I do agree that smoking causes cancer, the epidemiology is pretty solid on that. When it comes to second hand smoke however, I think we are getting a lot of want-it-to-be studies again. I already stated that I hate smoking, will not allow it in my house. Nevertheless, science is science, and it is not a good idea to conduct or interpret at studies with a predetermined outcome in mind.
