The only caveat I'd add is that any clear cut negligence should not go unpunished. The problem is this, only by co-operating to get the truth, and by assuring those being questioned that there is no penalty will you get the honest answers and the truth you need to understand. As soon as you say, negligence will be punished, the fearful will say nothing and the manipulative will endeavour to move blame to them. So if you go in, and say we are going to study this, and need your help, not to find guilt but to understand the events and reasons for this, and to see how and what you needed as tools that you do not have so it stops happening.
