Entitlements, all welfare programs. I'm not sure if it includes other things, but even if it does they would only make a minor difference.

Does the defense figure include spending for the Iraqi war

I think it does include Iraq, but if it doesn't, adding Iraq only makes a marginal difference. It wouldn't change the overall point of that graph. Defense used to be the majority of federal spending, its gotten much smaller by that measure (or by % of GDP) and the recent increase, is only a blip against the larger trend in the other direction.
