My brother bought a $100 gift card at a Wal-Mart last August, and the checker palmed it and gave him a "blank" card. He unwittingly sent it to my parents for their anniversary, and they discovered it had never been filled when they tried to use it.

My brother had his receipt and was able to get the card replaced, but the store manager chose not to pursue the theft even though the employee was marked on the receipt and still worked there.
