There is choice in that one can go private with suplimental coverage.
bottom line ...everyone has insurance...those that can afford it can by extra insurance privately.
There are lots of private clinics.

I don't think those leaders who descend upon us are generally from France or England, or the Scandinavian countries or Germany. Don't the people who come to the US for health care tend to be rich leaders of poor, non-Western, countries?
