A couple of reasons I think:

-There are well over 200 "insurers", all with different billing systems and rules. Dealing with the varied systems costs them tremendous overhead.
-The HMO's and insurance companies are in the business of making a profit by denying claims. They are not in the business of "healthy patients".
-When they treat an uninsured patient they probably get stiffed.
-It would probably be easier for them to lobby one entity (the government) for positive changes than trying to change all the health insurance companies.
-They probably believe everybody should have the ability to be treated; it would be good for their business volume!
