Whoops, sorry about that. Senior moment.

You asked what I would do. If I couldn't afford to have kids, I wouldn't have them. Affording to have kids means having health insurance coverage that will cover triplets, particularly if multiple births run in the family or if one is undergoing fertility treatments, or Medicaid or some such pre-arrangements. Not knowing the risk in this case, it's hard for me to evaluate any pre-conception or pre-birth irresponsibility.

Anyway, you don't design systems around exceptions. You design them around the mainstream.
