You can probably find thousands of examples where the savings aren't worth the time or effort. Try looking from the other end and I'm sure you can find examples where they are worth the effort.

Sure, there are. But we're talking about adding some overhead that isn't there now. Right now there are no such discussions. If even ten percent of visits provoke a discussion, that's a pretty big burden. There may be a commensurate savings, but at the very least it would jigger the ratio of doctors to patients. When we talk about medical savings account and people paying their own bills, this is a cost that I have never heard discussed. It could be a significant cost. It sure would be a change.
