I got the impression that the junior captain who was initially assigned to do the report wasn't the sharpest knife in the drawer, he was just the officer who could be spared from the unit's combat duties to do the initial investigation. His DRAFT report disappearing isn't especially unusual, it went through many hands before it was finalized and submitted to his battalion commander.
