With firms like Blue Cross or Blue Shield, I've seen

Doctors typically receive 55% of their billed amount, a 45% discount

and Labs receiving 42% of their billed amount, a 58% discount.

Not surprisingly, my Doctor accepts 50% as a cash up front payment in lieu of insurance - which also reduces billing costs.
