Assuming the report is accurate, and that I understand it correctly - It could be corruption, or incompetence, or a general system failure where competent honest people can't easily prevent mismanagement. That last could be entirely a failure in the bureaucracy, a failure in the systems they use, it could be a problem with the regulations and laws they are supposed to apply, or it could be a combination. I think this sort of thing probably is and has been more common than most people believe.
