SS and Medicare spending have a dedicated tax that wouldn't be there if not for the program.

The two where created together but the tax is still a separate thing from the spending, and either could be canceled without canceling the other, just as the federal income tax could be renamed the Iraq war tax. None of those things are at all likely to happen, but the name of a tax, or the fact that a tax was brought in at the same time of, and/or because of some spending category, doesn't make the spending different from other spending.
