Reducing the supply of new doctors is one of them. Another is increasing the percentage of doctors that refuse to accept new Medicare patients. These are both results of government efforts to contain costs by paying ridiculously low reimbursements.

Most people who understand the risk of empowering entrenched government bureaucrats to deny them health care would never consciously choose such a system.
