There probably are. If there where not risk aversion would likely be an extreme problem in such organizations. As it is there is still a lot of it.

As I said earlier, faceless functionaries, except in the case of the occasional crook or nut, go with the flow.

I agree with that. The usually tendency is to go with the flow. Nuts, or those with a different agenda probably won't do so well in these types of organizations.

The problem isn't as much some minor "faceless functionary" deciding to deny care on his own initiative, but rather people higher up denying care as a matter of policy.

And if they don't deny many forms of care, then the problem is that people have to pay a lot to get such gold plated care.
