No, I don't expect them to operate at a loss. Car insurance now has special pools for high risk drivers. That's one option. But the best option is to buy your insurance before the risk is discovered. That's a problem now because when people change jobs, they change insurance. If the insurance were independent of the employer, you'd buy in when you're young and risk free and stick with it.

Is there something unique about the US that would make exclusion more likely?

First of all, if the cost is perceived to be low enough, there's not as much pressure to go after the outliers. Other countries seem to tolerate high taxes more than we do plus other countries have cheaper health care. And second, we have a buttinski culture that is predisposed to legislate against behaviors. For example, prostitution is legal in most of Europe.
