Medicine will always have at least two tiers, if not in this country, then elsewhere. I just got through reading "Patients Beyond Borders" which is a guide for "medical tourists".

Any American can now travel to a foreign country and have anything done by a world class physician for less than it would have cost here, from a doctor that probably trained and practiced here and is nationally and internationally certified.

Most of these countries have no government health care, and the foreigner is taking time that would have gone to a less well off native, and paying more. That's just how it goes.
