So, then, how would we go about not paying for the R&D? Price controls? Or government direct purchase of the drugs and establishment of a monopoly distribution system? Or what?

A start would be to allow Medicare to negotiate drug prices. If we had a national system then that system would have even more buying power.

I often wonder, when you pay your co-pay of $10 or $15 for a prescription for a $100 retail drug, what the insurance companies are actually paying for the drugs vs. an uninsured consumer. And vs. Medicare. The insurance companies don't have any restriction on negotiating drug prices... I suspect they pay closer to the European system prices than the US retail price.
