The rationing was based on weight.

If there were no shortage of organs, then weight would not have been an issue. We'd transplant hearts into fat, alcoholic geezers. Right now, any of those factors can keep you off the list. You can always squeeze out another few hundred K for a liver transplant for a 17 year old girl or even for a fat, alcoholic geezer. Squeezing out another liver is a deal breaker.
