So, if it's not "the bottom line of the insurance company" but rather the bottom of the taxpayers' pockets, do we pay for experimental treatments or treatments unlikely to work or not?

You are looking for a black and white answer to a grey question. I am not a medical ethicist but such beasts do exist. Doctors make those decisions often, as do next of kin.
