I can't give you a personal example since moving to the Chicago area as I have not used the insurance yet. I can tell you when I lived in the suburbs north of NYC since I didn't have ANY choices to grab as a self employed individual, I went with an agency that apparently pooled a bunch of us, and chose some insurance carrier that was not a major. When we finally used it, the charges were uniformly rejected, the agent said 'I'll handle it' and kept resubmitting them, and after a year of this I started to get dunning notices about collection agencies. The agent ended up suing the firm and I don't know the result as I was in the process of moving and ended up paying the charges just to get them out of my hair.
