Also I would add that there are a lot more than 16 different factors, there where only 16 (if that was indeed the exact number) factors mentioned in an earlier post. There are others that I believe have been mentioned here, still more that we could come up with, and than there's unknown factors as well. Which increases the likelihood that they balance out. Whereas not having universal coverage is unique to the US in the sample, and as you admit effects longevity for those not covered. I'm not sure what's so complicated.
