The approx 70% of Canada's health care costs that are considered to be medically necessary are hospital and physician services and are funded almost entirely through general tax revenues of the provincial and federal governments. There is no specific premium as such. However some provinces require employers to pay a payroll tax and some collect a tax from employees on their earnings. I'm not sure about the self employed.
