The ratio between the amount hospitals charged self-pay patients (gross revenue) and the amount the hospitals actually collected (net revenue) was 2.57 in 2004. This means that hospitals collected only $0.39 for every dollar charged."

One, as you probably know, the ~40% is on total collection... and many uninsured who have to visit an emergency room can't afford to pay a dime (and never do). So 40% end up paying their billed price, or maybe a small discount. Or are you claiming that every single uninsured hospital patient pays... with a 60% discount?

Frankly that proves my point.

I submitted real data from a real bill. If you choose to believe that the companies for some reason put fake numbers on the bill then could you explain the mechanism by which they lower the cost? And if they actually charge less, why are they billing more? It doesn't make sense, does it?

Unless you can explain all this to me... then I'll assume that the information just doesn't fit with your ideology, and so you refuse to face reality.
