We have enough capacity to serve our public most of the time but situations arrive where it makes sense to pay top dollar for American treatment.

BTW, "top dollar" merely means what they charge everyone else who hasn't negotiated a lower rate - as some insurance companies do.

As i have said, your system is quite happy to have a customer that always pays its bills at the top rate

I'm sure. Plus there would be a moral problem turning away a pregnant woman or other person needing quick care.

I think free markets work best too.

As for the uninsured, that doesn't mean denied care.

As for denied claims, that happens in socialized systems too. Maybe not explicitly but implicitly, sure.
