Does your policy have an annual cap?

If it does, I can't find it. This is what it says re catastrophic benefits:

"Catastrophic Benefits

100% payment level begins after you pay $4500 out-of-pocket in coinsurance, copayment and deductible expenses"

This is what the glossary says:

"Lifetime Maximum.
The maximum amount the Plan will pay on your behalf for covered services you receive while you are enrolled in your option. Benefit amounts accrued are accumulated in a permanent record regardless of the number of enrollment changes. See Section 5(e) of this brochure."

Section 5(e) is the lifetime max for mental health and drug treatment.

I find nothing indicating any annual or lifetime maximum for other ailments.

I'm surprised. I remember clearly that there was a lifetime maximum back when I first signed up for the plan thirty some years ago.
