One suggestion for those who object to requiring helmets is simply to require prepayment in full for all medical care on injuries resulting from failure to wear a helmet. The US medical system provides millions of dollars of unreimbursed services for such injuries.

Sounds good. That ought to increase the cost of a cycle if you don't have health insurance to about $1,000,000. :-)
