Thanks for the link. I guess that number is stable from the nineties. It's definitely not "almost all."

Some can legally charge more of the patient than Medicare will pay.

I thought they all could now? Am I missing something? I haven't had occasion to see my doctor since I entered Medicare next month but, when I do, the first thing I will tell her is that I'm prepared to pay her extra for her services if need be.
