Yes, the purpose for free stuff is to sell stuff. And I suspect you are correct that your doctor has a sense of charity and assists patients perceived to be in need in ways that may not always be accurately measurable.

For instance, a doctor may choose to bill some patients for a simple procedure when the visit qualified for a complex one. That would of course show up on the radar of freedom haters as proof that the poor are receiving lower quality care.
