In Canada, the point is, for health care,if you make out a tax form you are probably paying your share or at least something. There is no choice to free load, and have a "choice" to pay nothing.

Those in lower tax brackets would pay less, example young people just starting out and senior citizens on fixed incomes.

You never get complete fairness in any endeavour, at least all taxpayers are contributing and yes the unfortunates, homeless, working poor, addicts, etc. are looked after..i just consider them unfortunates "with a bad ticket in life(Buffet)" and not weasels choosing not to pay.
So have a tax system that is fair, because it is the tax payers of a country that pay for all the ""commons," like it or not.

I have been posting generally about our system..we aren't billed for doctors visits or specialists visits...if you get sick and land in the hospital you will not face giant debt burden or bankruptcy..just present your health card and you will come out the door financially whole.

There are a few tests that we have to pay a portion for, not to many.A large portion of cost of drugs are covered for senior citizens.Drugs outside the hospital are not covered so you must buy a plan or pay outright for them.
Dental, pulling, filling teeth etc. you have to buy a plan or have a plan through work or just pay outright.Major dental done in a hospital i believe is covered.
