During the last down-turn forensic auditors at the RTC almost always found fraud was a major contributing factor of the bank's collapse.

Is this related, or is fraud rampant at most banks?

You're correct. It would take exceptional greed and stupidity. Fortunately, we have plenty of that around in our banking/financial system.
