The first priority is to get the cost down through structural changes.

Your choice not mine. Not "the" but "my."

If you deal with the uninsured first, demand goes up and so do costs. Much tougher and much less likely to get the structural changes done later.

There is a moral issue here which transcends the fiscal one.
