I don't think either health care, or more relevantly health care insurance is homogeneous. Insurance can be structured in different ways, and sold in different ways, and different companies can be more or less reasonable about how they handle payment disputes. Getting a reputation for being very tough and unreasonable would get a different reaction from future buyers than getting a more reasonable reputation.

Also even companies that sell truly homogeneous products try to develop positive reputations to the extent that they can do so.

Also your statement wasn't limited to insurance companies -
"Like any profit-driven entity, it must strive to deliver as little as possible while taking in as big a payment as possible."
