Not entirely. It's now clear that he was blindsided by the leak of a distorted report of a meeting between Canadian officials and one of his aides. The full truth of the matter (along with the Canadian Prime Minister's admission on the record in the Canadian Parliament that the memo was "blatantly unfair" to Obama) did not come out until after the primaries.

Given the exquisite timing of the smear memo leak - just enough time to have the desired effect, but not enough for any full rebuttal to be prepared - strongly suggests (to me at least) that it was deliberately planned.

I don't think the Clinton campaign were part of the planning, but they were entirely happy to exploit the issue at length, in the knowledge that they were guilty of exactly the same double dealing with which they accused the Obama campaign.
