What about -

1 - The data is stored in a secure central location, that generally is NOT easily available (no internet access, no dial in ports)

2 - Anyone who signs a release form (perhaps a notarized one if there is too much concern about faked forms), gets the data mirrored on to a server that allows secured access from the outside.

3 - If you don't allow this access (or I suppose even if you do, but you want your own copy), you get a digital copy of the information. Might be the size and shape of a credit card, it might be available in other formats as well. Depending on the type of security on the card, and the type of reader needed to access it, it might be something where you could make your own copies (say an encrypted copy on your computer, or a backup card).

4 - Doctor's offices, hospitals, ambulance's etc. would have access to the secure server, and a reader for the card.

It would seem Peter gets what he wants and you get what you want.
