No idea if the costs are distributed equally in group health policies, or a premium charged in states with high medical expenses.

The States mandate what must be sold and restrict the companies that can sell it. You mentioned Mass. Because of their mandates, individual policies, now required there, cost three times as much as the they in Texas. So if you live there, the best thing to do is go on the State program. Which will be bankrupt by next year.

A Federal law requiring all states to allow the sale of all health policies without restrictions would solve this.

Don't hold your breath.
