I'm familiar with the occasional dissonance between gold and stocks you describe but my feeling is it happens fully only a few times a year usually at turning points or when the general market crashes like last August (my regression model for the HUI based on gold had a residual of negative 10-15% at that time - about as low as it gets).

As well as the things you mention I think about 40% of the S&P earnings come from overseas which helps explain their outperformance relative to the microcap index IWC:
