Maybe for the first five years, only cover one annual physical

Medicare currently only covers one physical. Not one annual but one lifetime. And then only within the first six months of enrollment. I need to hustle to get mine.

the last few months of a person's life when their systems are breaking down

They've already shifted into active support of hospice. They don't push you into it, but they make it almost too attractive to turn down. That's fairly new, I think. My dad jumped at the chance once I had explained the options several time and he understood it. I'm not sure how many elderly have family that can or will do that. Medicare and I both made the choice of hospice easy for him.

Physical therapy seems a license to steal to me, too. As long as you're improving, you get as much as you want.

BTW, my friend didn't take them up on their offer of the ED pump. He did enjoy the dirty phone call, though.
