Kate, the cut off for means-testing is 80K for individuals. The article on McCain and Part D made clear that means-testing was being applied to the "wealthy" as have other statements on means testing.

Yes, the article gave only one example, the couple example, but I don't see any way to conclude that "wealthy" somehow doesn't apply to individuals who also meet the means-testing criteria. That parsing is just too cute.

Do you think that $80K in taxable plus tax sheltered income appropriately defines the "wealthy" threshold?
