we would be well to look to Japan.
but the article fails to emphasize enough (though it does enumerate) that the real challenge comes from the inverted pyramid demographic shift in japan. the level 2 hospitals are in rural areas that cannot sustain doctors for a whole host of reasons, possibly most importantly because those areas have too few young families to maintain a decent school system.
