The term is used most frequently, and often pejoratively, in the U.S. political debate concerning health care.

I understand that the term is sometimes used pejoratively in the US. It's too bad that words get co-opted that way, particularly when there's no synonym. In this case, I don't know of an synonym. If you do, please share. I will be happy to use it even though that shouldn't be necessary. Outside the US, the word is an ordinary adjective.
