The military would come to a complete halt if it weren't for the extensive use of checklists. What also matters is the way in which you use a checklist. Reader states the action, hears a verbatim repeat back, operator initiates the action and reports step complete.

Sounds simple, but many in civilian industries believe it is a waste of time. In the military it saves countless lives. How else can you explain hundreds of 19-21 year olds moving massive stockpiles of weapons around safely, when a few months earlier they struggled to give you the right change at Pizza Hut?

Educated hospital workers often think they are too smart to need such training aids. Culture is largely the problem.
